Relocation Forms

If you have a mobile MRVP voucher, you may be able to relocate to a new apartment while keeping your housing subsidy. Please note: Before you can move to another unit and continue to receive your housing assistance, you must give proper notice to your landlord as is detailed in your current lease. Failure to do so may lead to termination of your rental assistance.

The following forms are used during the relocation process:

MRVP Relocation Packet

This packet must be completed by the participant/voucher holder and the prospective owner after the participant has been accepted for a suitable unit.

Reinstatement of Lease

This packet must be completed by the participant/voucher holder and the prospective owner after the participant has been accepted for a suitable unit.
